Notes:

• The shorter the survey, the faster the response rate.
• There are 3 types of questions: closed, open, mix between closed and open.
• Closed questions (choices) provides an easy method, but it should be comprehensive.
• Open questions to express the opinions and beliefs of the participants, but try not to be too broad in answer.
• Reducing error and bias is a more important goal than a high response rate.
